To change the death beneficiary for a Coverdell Education Savings Account (CESA), do the following:

1. In Xperience > SilverLake > StreamLine Platform Menu > Recall Existing Account from Core. 
2. Click OK.
3. From the Application drop-down menu, select T = Time Deposit.
4. In the Account Number field, enter the account number. 
5. Click Tab. The Branch field fills in. 
6. In the Platform Type field click the ellipsis  .
7. Choose *MD MISCELLANEOUS CD DOCUMENTS.
8. Click OK on each screen until you reach the MISCELLANEOUS CESA IRA FORMS screen.
9. In the CESA Designation/Change Beneficiary field, type Y. 
10. Click OK.
11. Complete sections 1, 2, and 3.
12. Print documents or use eSign to have the responsible individual sign and date section 4. See eSign for more information.
13. Have the associate changing the death beneficiary for the CESA account sign and date the Signature of Custodian/Trustee field.
14. Place the CESA Designation/Change of Beneficiary form not submitted through eSign with the daily work for imaging following the Document Imaging Procedures.
