A Survey/ Order Survey Form/ Survey Affidavit is a drawing of property boundaries that includes structures, encroachments, or easements. The survey should comply with the ALTA 2016 Minimum Standard Survey (ALTA/NSPS 2016) requirements.
The Order Survey Form is required when:

The loan is secured by real estate.

Exceptions:
- Vacant lot in platted subdivision
- Condominium unit
- Second lien position HELOC
- 1st Lien HELOC <$250,000
- Within platted/recorded subdivision
- Review of GIS and legal to confirm access

Georgia Real Estate:
- You may use the most recent plat when:
- Legal description does not include specific measurements and angles (for example: run South 2588 feet, West 38.48 feet, North 26 degrees 33 minutes West 279,30 feet) and refers to plat of survey
- Land is vacant OR
- All structures on the property are included on the plat
- You need a survey when:
- Legal description includes specific measurements and angles (for example: run South 2,588 feet, West 38.48 feet, North 26 degrees 33 minutes West 27,930 feet)
- There are structures on the property not identified on the plat
- You may use the most recent plat when:
An existing survey may be used if:
- Less than 20 years old
- Approved for use by the closing agent
- A Survey Affidavit is obtained
New surveys must be certified to:
- Capital City Bank
- Borrower/Property Owner
If title insurance is ordered, must also be certified to:
- Title Insurance Agent (for example: John Smith Title or John Smith, Attorney)
- Title Insurance Company (for example: Chicago Title Insurance)
Once completed, the Survey/ Order Survey Form/ Survey Affidavit is uploaded to Synergy (consumer purpose) or Abrigo (business purpose). If you need additional assistance with Abrigo, please contact _Abrigo Help.
Note: If survey is too large to upload, interoffice mail to Loan Servicing-Records and include the loan number.
