What Documents Will Get Us Started
Name Change
Name changes are made when clients provide proper legal documentation such as:
- Marriage certificate
- Court order
Clients must execute a new signature card and obtain the signatures of other authorized signers on the account.
Power Of Attorney
- Signed letter requesting what is needed - letter must state where the principal is and why he/she can no longer act independently
- Power of Attorney documents
- Valid ID
Trust
- Signed letter requesting what is needed
- Trust documents
- Valid ID
Representative Payee-SSA
- Signed letter requesting what is needed
- Letter of Authorization from the Social Security Administration or advise reflecting Representative Payee status
- Valid ID
Representative Payee-VA
- Signed letter requesting what is needed
- Veterans Custodial Agreement
- Valid ID
Guardianship
- Signed letter requesting what is needed
- Original court order
- Valid ID
Estate
- Signed letter requesting what is needed

Special Provisions For Alabama And Georgia
Contact CMD for guidance and instructions when a request is received for the payment of account balances for interstate depositors. There are provisions for the disposition of funds when the depositor dies without a will and had $5,000 or less on deposit in a bank in Alabama or $10,000 or less on deposit in a bank in Georgia.
Accounts With Power Of Attorney Or Rep. Payee
The POA/Rep. payee is null and void when the principal dies. Do not allow the Attorney-in-fact/Rep. Payee use the Power of Attorney (POS) / Rep. Payee to transact business on the account. Refer to the Power of Attorney / Rep. Payee section.
Deceased
Death of the individual account owner:

Guardian Accounts
Contact CMD for further assistance with Guardianship accounts. Guardianships are null and void at the ward's death. Do not allow the guardian to use the account after the ward is deceased. The court appoints a new guardian upon the death of the guardian.
Representative Payee
Upon the death or revocation of a representative payee, the Social Security Administration appoints a new representative payee.
Sole Proprietorship Accounts
Any additional signer's authority ceases at the death of the sole proprietor. Access to the account is only granted after the proper legal documentation has been provided. Documentation includes:
- Letters of Administration
- Death Certification
- Valid ID
