Affidavit Completion
Proper completion of the affidavit is required in order to not be placed on the Document Exception Report (DER). Guidelines to assist with proper completion of the affidavit are below:
Fields
A. Enter the applicable state where the document is being notarized.
B. Enter the applicable county where the document is being notarized.
C. Enter the name of the affiant who is signing the affidavit.

H. Enter image date of previously imaged documents of enter N/A if referenced
documents in fields F and/or G are attached.

Notary Requirements
The CCB Affidavit is to be notarized by a notary based on state requirements.
As an exception, if all notaries are out of the office, the affidavit can be witnessed by 2 associates and indicated on the form. Email _SupportU to notify of this exception.
