Completing the Stop Payment Request Form

To complete the Stop Payment Request form, do the following:

  1. Ask the client to verify the information on the Stop Payment form.
  2. Obtain the client’s signature and the client’s initials to authorize the expiration date.
  3. Provide a copy of the form to the client.
  4. Inform the client that the stop payment fee will be debited from his/her account.
  5. Upload the stop payment in Streamline.
  6. Place the applicable documents not submitted using eSign with the daily work for imaging.

Note:

  • The signed stop payment request must be received within 14 calendar days of the original request.
  • The client can send the signed written stop payment request by mail, fax, or in person.
  • Deposit Services is responsible for reviewing the account after 14 days to ensure the stop payment was signed or the written request was obtained.
  • If the client does not provide the signed stop payment form or written authorization for the stop payment, the stop payment will be deleted .
  • Do not refund the stop payment fee.
