The entrance log is used for entry into the main vault, separate Safe Deposit Box (SDB) area, or any other area requiring documentation of entries. All entries must be documented following requirements for dual control or non-dual control and when assisting clients with SDB access. Cameras do not replace associates for the dual control requirement.
An authorized associate is an associate who is listed on the Key/Combination Distribution List for access to the dual control area.
To protect all parties involved, strict adherence to all dual control practices are required.
Completion of Entrance Log
Fill out the Entrance Log with the following:
- Mark an "X" to indicate Vault Log, Safe Deposit Box Log or Other .
- If Other, write the log title (for example: dual control safe box).
- Write the year, month and office location in the spaces provided.
- Upon entry, write the date and time of access on the log.
- Mark the applicable column with an "X" to indicate the access reason.
- For access reasons with (*) or (**), you MUST enter the required additional information on lines 4 - 12.

Open Vault
When opening the vault, dual control is required. Record the following information on the entrance log:
- Day (1, 2, 3)
- Time
- Mark an "X" in the Open Vault column.
- First initial/full last name of associate 1
- First initial/full last name of associate 2
Close Vault
When closing the vault, dual control is required. Both dual control associates must be actively involved in setting the number of hours until the vault opens. Associates must check the vault room, privacy room (if applicable), and general vault area for occupants prior to closing the door.
Record the following information on the entrance log:
- Day (1, 2, 3)
- Time
- Mark an "X" in the Close Vault column
- Enter the number of hours set until the vault opens in the Number (#) Of Hours Set for Opening column.

Teller Drawer Removal
Record the following information on the entrance log:
- Day (1, 2, 3)
- Time
- Mark an "X" in the Teller Drawer Removal column.
- Teller drawer number
- Signatures as follows:
- When drawers are located behind a dual control door, do one of the following:
- Record the first initial/full last name of of both authorized dual control associates that retrieve a teller drawer
- Record the first initial/full last name of both associates who allow and observe access of other associates retrieving teller drawers.
- When drawers are located in a non-dual control area:
- Single control access is permitted.
- Record the signature of the associate who retrieves the drawer.
- When drawers are located behind a dual control door, do one of the following:
Teller Drawer Return
Record the following information on the entrance log:
- Day (1, 2, 3)
- Time
- Mark an "X" in the Teller Drawer Return column.
- Teller Drawer number
- Signatures as follows:
- When drawers are located behind a dual control door, do one of the following:
- Record the first initial/full last name of of both authorized dual control associates that retrieve a teller drawer
- Record the first initial/full last name of both associates who allow and observe access of other associates retrieving teller drawers.
- When drawers are located in a non-dual control area:
- Single control access is permitted.
- Record the signature of the associate who retrieves the drawer.
- When drawers are located behind a dual control door, do one of the following:
Safe Deposit Box Entry - CLIENT **
Record the following information on the entrance log:
- Day (1, 2, 3)
- Time
- Mark an "X" in the Safe Deposit Box Entry - CLIENT ** column.
- First initial/full last name of the associate accompanying the client to their Safe Deposit Box.
- In the Comments section, enter the following:
- Client's full last name or Business Name (for business client boxes)
- Box number
Safe Deposit Box Entry CLOSED BOX**
Record the following information on the entrance log:
- Day (1, 2, 3)
- Time
- Number of keys being returned
- Mark an "X" in the Safe Deposit Box Entry - CLOSED BOX ** column.
- First initial/full last name of the associate accompanying the client to their Safe Deposit Box.
- In the Comments section, enter the following:
- Client's full last name
- Box number
Safe Deposit Box Entry - CLIENT ASSISTANCE**
This entrance log column is used to indicate ALL access to a Safe Deposit Box (SDB) where a client requires assistance (for example: elderly, disabled, injured). Dual control is required when assisting a client access to a SDB.
Record the following information on the entrance log:
- Day (1, 2, 3)
- Time
- Mark an "X" in the Safe Deposit Box Entry - CLIENT ASSISTANCE ** column.
- First initial/full last name of BOTH dual control associates accompanying the client to their Safe Deposit Box.
- In the Comments section, enter the following:
- Client's full last name
- Box number
Safe Deposit Box - OTHER**
This entrance log column is used to indicate ALL access to a Safe Deposit Box (SDB) for box drilling, box repair or bank use (for example: CCB spare keys, official checks, dual control safe box).
Record the following information on the entrance log:
- Day (1, 2, 3)
- Time
- Mark an "X" in the Safe Deposit Box Entry - OTHER ** column.
- First initial/full last name of BOTH individuals accessing the area (for example: dual control associates, 1 associate and 1 outside vendor).
- In the Comments column, enter a brief explanation of the access reason.
Cash Vault Entry - CURRENCY/COIN
Record the following information on the entrance log:
- Day (1, 2, 3)
- Time
- Mark an "X" in the Cash Vault Entry - CURRENCY/COIN column.
- First initial/full last name of BOTH authorized dual control associates.
- Armored courier DOES NOT count as a second party for dual control requirements.
- A Lead Teller (LT) may access under single vault control. The first initial/full last name of the LT is required.
Office Items - DUAL CONTROL REQUIRED
This entrance log column is used when office items are stored anywhere other than in a Safe Deposit Box.
Record the following information on the entrance log:
- Day (1, 2, 3)
- Time
- Mark an "X" in the Office Items - DUAL CONTROL REQUIRED column.
- First initial/full last name of BOTH authorized dual control associates.
Key/Combination Box - DUAL CONTROL REQUIRED
Record the following information on the entrance log:
- Day (1, 2, 3)
- Time
- Mark an "X" in the Key/Combination Box - DUAL CONTROL REQUIRED column.
- First initial/full last name of BOTH authorized dual control associates.
- In the Comments column, enter a brief explanation of the access reason (for example: issuing key to new associate).
Other Entry - COMMENTS REQUIRED
Record the following information on the entrance log:
- Day (1, 2, 3)
- Time
- Mark an "X" in the Other Entry COMMENTS REQUIRED column.
- If dual control is required for the entrance reason, record the first initial/full last name of BOTH authorized dual control associates.
- In the Comments column, enter a brief explanation of the access reason.
Retention Period
The Entrance Log must be retained for a period of 6 years after the last entry.
