IRA Transfer, Direct Rollover, Conversion, or Recharacterization Contributions

A completed Individual Retirement Account (IRA) form is required for each of the following contribution transactions:

  • IRA Transfer-  IRA request for Transfer Form
    • IRA funds transfer from one financial institution to another
  • Direct Rollover - Direct Rollover Form - 
    • Withdrawing money to reinvest within 60 days
  • Conversion - Conversion Form
  • Recharacterization- IRA Recharacterization Notice 
    • Can recharacterize from Traditional to Roth but not from Roth to Traditional
    • Record the New CCB Account Number for the investment chosen

See New IRA Plan or Contributing to an Existing IRA Plan to complete the application form for printing purposes.  

Clients with an IRA request for transfer, direct rollover, or conversion require completion of the appropriate form. Once the form is complete and printed, mail the second and third copies to the financial institution for IRA funding to be released to CCB.   

  • IRA Request for Transfer, Direct Rollover, or Conversion Form
  • IRA Recharacterization notice
  • If the client is 70 ½ or older and is transferring a traditional IRA plan to CCB, complete an IRA Age 70 ½ Election Form
